Form submissions on this website are processed through Netlify, the hosting provider for this site. Netlify receives and stores form submissions on our behalf so that our team can review and respond to them.

Sensitive information

Please do not submit medical records, Social Security numbers, government identification, or other highly sensitive information through the forms on this website. These initial forms are designed to collect only the information needed for a first conversation.

How your information may be used

We use the information you submit only to respond to your inquiry and to communicate with you about the path you selected. We do not sell your information.
